The Thornhill Community Health Centre (CHC) is a new, not-for-profit primary care organization being developed in the Western York Region as part of the Reena group of charities. The CHC represents a long-term investment in accessible, coordinated primary care for individuals and families who currently face barriers to receiving consistent, high-quality healthcare.
The CHC has been approved for public funding through Ontario’s Primary Care Action Plan, confirming its role as a key component of the region’s healthcare system.
Thornhill–Vaughan is a large and growing community with significant unmet healthcare needs:
- Over 9,000 residents are unattached to a primary care provider
- There is no local Community Health Centre or Family Health Team
- A growing seniors population (~ 22% over the age of 65) and a high proportion of frail elderly
- Highest levels of mental health and addictions in the Western York Region (10.7%)
- Individuals with intellectual and developmental disabilities (IDD), neurodivergent individuals, seniors, and caregivers have complex healthcare needs, experience fragmented care, and are more likely to visit emergency departments and be hospitalized
- Thornhill is a developmental services hub with organizations such as DANI, Reena Inc. and Kayla’s Children Centre all within 1 km
Within the Jewish community specifically, these challenges are compounded by:
- Aging caregivers supporting adult children with disabilities
- Chronic and complex health needs requiring coordinated care
- Cultural, linguistic, and religious considerations not consistently addressed in mainstream primary care settings
A Community Health Centre Model
The Thornhill CHC is built on Ontario’s Community Health Centre (CHC) model, emphasizing:
- Interdisciplinary, team-based primary care
- Prevention and early intervention
- Integration of health and social supports
- Equity-driven service delivery
- Emphasis on neurodiverse and vulnerable community members
The Thornhill CHC will provide a comprehensive range of integrated services, including:
Primary Care Services
- Preventive care and health screening
- Acute care
- Chronic disease management
- Minor medical procedures
- Maternal and child health
- Neurodiversity-informed primary care pathways
Mental Health and Counselling
- Individual mental health counselling
- Psychosocial support
- Crisis prevention and early intervention
Allied Health and Interprofessional Supports
- Nursing services
- Social work
- Occupational and/or physiotherapy services
- Pharmacy collaboration
- Care coordination and system navigation
Health Promotion and Prevention
- Health education and counselling
- Exercise and wellness programs
- Falls prevention and mobility support
Social Determinants of Health Supports
- Navigation of income, housing, and benefits
- Connection to community and social services
- Support for caregivers and families
For both categories, you must have an OHIP card.
