Frailty is not a fixed state—and new research highlights that individuals with intellectual and developmental disabilities (IDD) can experience meaningful improvements when care is informed by frailty assessments. This pilot project tested the Home Care-IDD Frailty Index (HC-IDD FI) to see how identifying frailty influenced care planning and outcomes for adults aged 40+. By fostering collaboration between the home care and developmental services sectors, the initiative demonstrated that targeted care planning led to positive changes in both physical and social well-being. Key recommendations from the project include sector-specific training, data sharing agreements, and formal collaboration protocols to ensure responsive, coordinated support for aging adults with IDD in the community.
