The long term care sector and community groups offer a continuum of services and programs for all of Ontario’s older adults with the goal of supporting them to live as independently as possible in the community.

The continuum includes recreation programs, hobby clubs, seniors clubs, elderly persons’ centres, home and community support services (e.g. visiting health professionals, personal care, homemaking and a range of community supports such as Meals on Wheels, Diner’s Clubs, foot care, telephone visiting and more)

The continuum also includes a range of residential options for those individuals who are unable to remain in their own home or current living situation. These include supportive seniors apartments, retirement homes and long term care homes.

Websites where you can learn more about these and other services are listed below. Access to services funded by the Ministry of Health and Long Term Care requires an assessment and referral arranged through your local Community care Access Centre (CCAC).

Access to Seniors Community Programs – Selections From the Guide
Provides information about seniors community programs and guidance on how older adults with a developmental disability can access them. The guide includes material geared to the needs of self-advocates, caregivers in developmental services and staff of seniors community programs. The guide looks and acts like a website. Presents a highly useful summary of information on legislation and regulations pertaining to substitute decision-making and financial issues. Provides information about Ministry funded programs for older adults and how to access the Community Care Access Centre.

Ontario Association of Community Care Access Centres
Provides information for professionals working in the field and for the public; site includes a CCAC Locator so you can find the CCAC in your area by entering your postal code; there are also links to some additional services and sites of interest.

Ontario Seniors’ Secretariat
Promotes recognition of the needs and concerns of seniors and develops policies and programs to ensure they can live free from harm and with dignity. The site includes a variety of information about various provincial government initiatives on behalf of seniors; you can access the Seniors Guide on the site; there is a link to “Seniors Info,” another site managed cooperatively by all three levels of government with a wealth of information.

Older Adult Centres Association of Ontario (OACAO)
This provincial association maintains a listing of all member elderly persons centres in Ontario; go to the Membership Directory button once inside the site.

Ontario Association of Non Profit Homes and Services for Seniors (OANHSS)
The OANHSS site offers weblinks to services and programs for seniors across Ontario; there is also a locator function to find services in your area.

Ontario Community Support Association (OCSA)
This site has a locator to help you find programs and services in your community Care; also a link to the Guide to Help People Live at Home.

Ontario Long Term Care Association (OLTCA)
The OLTCA website offers information about making the decision for a long term care home and has a search function to find long term care homes in your area.

Canadian Association for Community Care
This site includes information about programs for seniors; resources for care givers and a manual for the safe use of medications with seniors.

Hospice Association of Ontario
This site provides information about end of life issues and services; you can also access their Directory of Hospice Palliative Care Services

Seniors Canada On Line – Government of Canada
Information source for seniors, caregivers, families and service providers on services and resource material on many subjects related to aging

Victorian Order of Nurses (VON)
Provides information on healthcare issues and resources for caregivers
